Pathways vermont transforms the lives of people experiencing mental health and other life challenges by supporting self directed roads to recovery and wellness in an atmosphere of dignity, respect, choice and hope. Housing first program: pathways vermont's housing first program provides permanent supportive housing services to individuals with long histories of homelessness and institutionalization who are living with significant disabilities. The program uses a scattered-site model, assisting clients to locate independent apartments leased by private landlords across the community. In addition to housing placement, the program provides wraparound community support services to assist clients in defining goals and overcoming barriers to independent living. These services include multidisciplinary approaches including benefits coordination, case management, psychiatry, substance abuse recovery support, employment support and nursing. At the end of 2017 the program has found permanent housing placements for more than 400 households.
