The community health centers of Burlington (CHCB) is a nonprofit health and human services organization whose geographic reach spans Vermont's most populated and urban county, Chittenden county, and the rural southern portion of grand isle county, representing a total of 150,000 vermonters. Through our 24-year successful history as our region's sole federally qualified health center, CHCB has served our community as a health care home for vermonters of all walks of life, regardless of ability to pay. The community health centers of Burlington (CHCB) is a primary care family practice and a federally qualified health center offering medical, dental, laboratory, psychiatry, mental health, substance abuse and case management services as part of an integrated medical home. CHCB offers a sliding fee financial assistance program for all services based on income within our defined service area, and to those qualified as homeless at no cost to the patient. CHCB serves all members of the community as patients regardless of life circumstances, income or insurance status.
